Legal Question in Credit and Debt Law in New York

For a debt settlement case, I need the language / text used in the Verification for an Answer (not discovery, don't need that) to a complaint in the state of New York. I need the language for when it is NOT Verified and the language when IT IS Verified. Any help would be much appreciated.
