Changing Middle Name after marriage

I had recently gotten married and would like to legally change my middle name as well as my last name. Is there a specific form that I would need to fill out? The marriage certificate does not include the option of changing my middle name and as a result does not show my new middle name. Will this be a problem when changing my name at the DMV, passport, social sec., etc.?

In New York a person may change their name by usage - as long as there is no fraud involved. However some agencies want to see a court order. There is a proceeding that can be brought before the Supreme Court to acknowledge a name change. If approved, the court will sign an order acknowledging the new name which can then be given to DMV, social security, etc.
