Legal Question in Family Law in New York
How do I get my daughters birth certificate amended to include my name as father. Not married to the mother.
Mother is in agreement.

If you want to change your daughter's name and establish paternity at the same time, file an Acknowledgement of Paternity. I've attached a link to the form -- https://www.childsupport.ny.gov/dcse/pdfs/4418.pdf
