Do you need a witness signature and date on a legal agreement?

On a confidentiality agreement should the document have a date, a witness and

be noterized to be a legal and binding document?

A date and signature is all that is needed. Initials on the relevant material by the viewer is also good.

The notary and or witness are not needed but does make the document easier to prove later.
