limited release form

I need to obtain a release from individuals to be able to use their image on a commercially produced poster that will be sold to schools. what is the name of such a form?

I've seen it referred to as a photo release, image release, model release, etc. You can easily find what you need with an Internet search. However, you should always seek advise from an experienced attorney when you're going to rely upon an agreement, which is what a release is.
