Independent Contractor Agreement

Dear Sirs,

I am seeking an Independent Contractor Agreement that specifically addresses the following items:

- Not an employee

- No company benefits

- No withholding taxes will be taken

- Responsible for your own taxes

- Responsible for your own insurance

- 1099 will be issued

I also need this agreement to be acceptable for all states in the US.

Do you have an Independent Contractor Agreement that specifically addresses these items?
