I am not familiar with the forms available on findlegalforms.com. However, if title to the home is in the name of both you and your wife, you would need to use a Bargain and Sale Deed with Covenants to transfer the property from Husband and Wife to Wife.

You will also need the proper tax transfer documents to be completed and filed simultaneously with said Deed.
