Involuntary Change of Executor

In what circumstances can an excecutor, who has not done their job be replaced involuntarily, and how do we go about that process?

You file a petition in the Surrogate's Court to replace the Executor; but it must be factually supported by assertions of illegality, incompetence, persisten refusal to act, self dealing, etc.
