Legal Question in Wills and Trusts in New York

I had a power of attorney completed prior to the increase in filing this document. I then paid approximately $50. The person I appointed power of attorney is no longer in my life and want to change the power of attorney to a family member. Will I have to go through the process again and pay the steep increase in fees or is there a simple form I can fill out to change the name on the form?

I would suggest that you retain an attorney for this. The form has recently changed and became is complicated. However, if you want to do it alone, you may obtain a form Power of Attorney through the New York State Bar Association website. Furthermore, you must properly terminate your prior Power of Attorney.
