How do I add my daughter's real father's name on her birth certificate when it says "Husband's Information Refused"? I was separated from my now ex-husband for 2 years when I had my daughter. He is obviously not the father so I refused to list him because they would not let me add her real father. Her real father did sign a form at the birth saying he is the biological father. Her real father and I are now married.

Generally, when 'refused' is marked on a birth certificate it isn't so much that you 'refused' to list husband as father. It's that the hospital refused to list biological father as the father because the law requires that a biological father not be listed if you are married. Since you have divorced and now are married to the biological father, you don't need to go through the formality of a DNA test. If the biological father is willing to be listed on the birth certificate, you need the biological father is sign an affidavit of parentage and then petition to have a new birth certificate.
