I live in North Carolina, how do I change my name back to the name on my birth certificate if I am not getting a divorce but just changing my name?

There are two ways: 1) the consistant and nonfradulent use of what ever name you choose and 2) a simple legal name change. The legal name change has the benefit of allowing you to change other important documents like your drivers license and social security card. Any compentent general practice attorney can do a name change fro you. $500 is the extreme high end as far as fees go. Any attorney charging more than $500 for a name change is basically ripping you off as the process is incredibly simple and would require about 2 to 3 hours or less of total attorney time to complete.
