Do I need a patent or legal permission to manufacture a t-shirt
(crew neck)with my designs on it?

Patents do not relate here; if you are creating original art for a T-shirt, then you would own the copyright in the art and can do with it whatever you wish. Others, without a license from you, could not use your original artwork.
