Legal Question in Family Law in Ohio

divorce question

My question is as follows: My X-husband and I was divorced in the State of Ohio in 1990, in March of 1991 we moved back in with one another, and we resumed as Mr & Mrs, we never re-married. we did not file Income tax together, that is the only thing that we did not do as Mr& Mrs. We signed credit cards as Mr & Mrs, our health Ins etc.... My X-husband and I split in 1995 and have not been together since this final split. A friend of mine told me that since we signed and resumed as Mr & Mrs that we are still married and needed to file for divorce again. How can I find out if this is true , and if it is what do I have to do to get this straighten out? I would appreciate any help on this matter. Thank you
