if you are hired as a nurse and if the place you work for wants you to work as a nursing assistant if you refuse can they fire you

If you are an at-will employee they can fire you for any reason or for no reason at all. You, on the other hand, can quit for any reason or for no reason at all. Therefore, if you are at-will, your employer can assign duties as he seems fit.
