Legal Question in Traffic Law in Ohio

I was recently in an auto accident and had to go to the hospital as a precaution. The at-fault driver's insurance company offered me a settlement, which was verbally accepted. They sent a form to be signed and notarized. I signed and notarized the form, but before I could mail it, I received yet another bill for medical services. Is there a way to void the form? Am I required to send it back to them? I can't afford to pay the additional amount on my own.

You are not required to send the form back to them. Have you signed anything else?

You should not settle until you know what all of your medical bills are. Additionally, there may be items that you can sue for.
