what is a sworn statement of evidence?
Our daughter was adopted in China
and re-adopted in Ohio 8 years ago.
OH issued the birth certificate with
the wrong birth date. It wasn't an
issue before, but is now because she
cannot get a US passport since her
foreign records have 2-1-92 as
birthdate, and OH birth certificate has
5-13-92. OH probate court wants us
to appear but we now live in SC. We
can send a ''sworn statement'' and
''statement of evidence'' instead of
appearing, but can't figure out what
this is.

A sworn statement most likely refers to the affidavit of someone who was present at the child's birth and can affirm the birthdate. Evidence would include birth records from the hospital, foster parent, agency, or orphanage that has any information regarding her birth records.
