Legal Question in Wills and Trusts in Ohio

I need a "transfer on death deed form with affidavit of TOD". I want to purchase

the correct form. The only form that I seem online form US Legal is a Transfer on Death Deed-Beneficiary Deed - Ohio (Individual to Individual) - Form number OH-019-77. My question is can I use this form for that purpose.

If you are trying to create a trasnfer on death ownership then you can use the language in the Ohio statute. http://codes.ohio.gov/orc/5302.22
