If your child was born in Oklahoma and you are trying to get your babies birth certificate, all you will need is your name and the name of the babies natural father and because you are only 16 it is recommended that you take your parents with you to get the certificate, just in case you need them. To get the certificate you will go to the Oklahoma State Health Department, Vital Statistics and fill out an application for your childs certificate. They will ask for information about you and the baby's father; no your parents and you will get the opportunity to take to someone or ask questions at that time. Once they locate the certificate, they will charge a fee for a copy.

However, if your child was born in another state, you will have to obtain his or her birth certificate in that state.
