Auto Accident Claim

I have been involved in a car accident on March 5, 2002. I was not at fault. My car was totaled (though it was on old one). I have suffered some minor injuries and bruises. The other driver�s insurance has been somewhat straight up with me. They paid the premium value on my car. They paid for rental car until they made an offer on the car (2 1/2 weeks worth). They wanted to make an offer to settle the case; I told them I wasn't ready yet. Now, I am ready to file. Since the insurance company hasn't been a pain, I am thinking about filing myself w/out an attorney. I am aware to include in my claim medical bills (prescribed and of the shelf one), lost of wages, mileage, and a figure for my pain and suffering. Is there anything else? Is there a form to fill out to present the claim to the insurance company? Thanks for your help.
