Is there a specific form that I should use when entering a written appearance personally in defense to the claims set forth against me in a civil law suit?

You did not mention whether you are in "small claims court" District Court or the Court of Common Pleas. If you are in small claims no specific answer is required but you must notify the Court in writing that you intend to defend. The other court requires complicate pleadings and you would ill served if you attempt to represent yourself. I strongly recomend that you obtain legal counsel in that event. There is no form to "enter an appearance." Only lawyers do that.

If this is a small claims matter, you should have received a "notice of intent to defend" form with the complaint, which you should file.

If this is a civil suit, you need to file an answer, etc. This is not easy for an untrained person. You should get a lawyer.
