My wife was granted full physical and legal custody of her nephew and I need a birth certificate to add him to my benefits on my job. Unfortunately many of the sites to order a certificate don't list the current relationship in order to order the certificate? any help ?

So why aren't the parents on the hook for the medical insurance for the child?
You have no family relationship to the child so I don't see how you can put the child on yor medical insurance.
This is an issue your wife's lawyer should have addressed.
