Legal Question in Medical Malpractice in Pennsylvania

in december 2011,the v.a. contacted me about the lack of care my father received while there, resulting in his death . they gave me forms to fill out asking me for a $ figure. i have no idea, what to ask for except that their people said to ask for a high amount. procedes and staff had been changed, but no one was punished legally.
