Perforation of the uterus should be an avoidable complication. This would be particularly true where the damage done required a hysterectomy. Nevertheless every case is dependent on particular facts which might be relevant to whether a case should be pursued. What was the condition for which the procedure was performed? What was the particular reason a hysterectomy was required rather than a repair of the uterus, etc.?

If you want to know if there was malpractice, you'll have to get an objective opinion from a doctor. You may need to go to an attorney to get that. Perforation is likely on of the possible complications, but that does not mean there was no malpractice. There are protocols set up that help establish the standard of care, records that must be kept, records that should be kept, etc., etc. You nor I can evaluate that, so, again, you must get a suitable doctor to review the records before you can make any decision regarding litigation.
