First, if you have homeowners insurance, the insurance company would generally have a medical payment clause that would pay for any injuries suffered by a guest in your house. In this case, however, the insurance company would probably deny coverage because your guest was involved in illegal behavior--underage drinking--and taking illegal drugs.

The major question, however, is whether you knew that the 20 year old was drinking and smoking pot in your home. If you did know, then you can possibly be held be liable for this person's injuries, whether insurance covers this or not. If you were unaware of this behavior, he would have a much harder time recovering from you.

If the medical bills are significant, you should at least give notice of a possible claim to your homeowner's insurance carrier. If you do not have homeowner's coverage, you should consult an attorney.
