Non-married couple dividing joint real estate and property

Is there a standard form to use for division of property between non-married people? In the event that my girlfriend and I split up, we want to make sure that the house is divided 50/50. We are both on the mortgage and have invested equally in the house. If no form is available, if we write are own contract or agreement and have it notarized, will it stand up in court or does an attorney have to draw up the contract?
