What exactly does a revocable trust agreement mean?

If it means that it can be changed then who has the authority to do so?

A trust is an entity with powers conferred by an Agreement. The Agreement gives the trust powers, and distinguishes how it will operate. A Revocable Trust normally can terminate at some point based upon action taken by the trustee or some other person given authority to cause a termination.
