Legal Question in Workers Comp in Pennsylvania

Work related death.

My husband passed away at the age of 28, in 8/06 after a 250-350lb object fell off the lift and hit him in his head and knocked him to the ground, he seemed ok at the time. His manager that day was made aware but did not send him to the ER. 3 days later he began vomiting blood and died. Doctors did a CT scan and noticed he had massive bleeding in his head. Can I sue his work, workers comp, or his manager? If so how long do I have to file a claim? My husband did not report this to workers comp., but there are 2 co-workers that see it happen.
