Legal Question in Family Law in Rhode Island

I went to court today for my divorce (neither of us are/were represented by a lawyer) and was advised that we needed to complete and file a particular form to complete the divorce proceedings. I was not preparded to take notes and was nervous and did not write down the name of the form. The judge suggested that we hire a lawyer to complete this form.

My question is:

What is the name of this form that we need to file and also what is the average cost for a lawyer to complete the form?
