Father listed as unknown on birth certificate, how to change?

2 year old daughter was born in Tenn. and I currently reside in North Carolina. At the time of birth, I listed the father as unknown. I do know who the father is and we are currently together and he has no question of the paternity of the child but wants his name put on the birth certificate, how do I go about changing this?

You will have to contact the Tennessee Department of Health- Vital Records (615-741-1763) to request the change. The father will have to sign a voluntary acknowledgment of paternity and there may be a nominal fee.

In Tenn. there is a procedure for legitimating a child and correcting a birth cirtificate. You should be able to do the former in NC but I am not sure ho to do the latter from NC. Please consult with a local attorney for details.
