Durable Power of Attorney Contested

How do I get a durable POA revolked, investigated, and/or transfered? Is there any law that requires the holder of a POA to notify the next of kin of the person whom the POA is over?

My cousin got my mother to sign a Durable POA while in the hosiptal 12 years ago since then mom has been in the hospital many times but no mention of POA was made until this last time when she had a stroke and was unable to respond to questions.
