Legal Question in Business Law in Texas

Hi,

I am from Texas, and have recently created a new cologne I would like to market. I would like to take recorded surveys of consumers trying the product and giving their honest opinion, and then use those recording as testimonials for my product.

This is actually similar to the Pepsi Vs. Coke test videos:

After the random testing, Selected customers will give their opinion on a 1 on 1 video, similar to this: http://www.origins.com/customer_service/video.tmpl

What type of legal contract should I get them to sign, so that I can use their video testimonial for my product online and protect myself? Is there a legal document/template that I can get online, that gives me permission to use their recorded video?

It doesn't have to be anything special.

It's a waiver. It should say:

"I understand that I was in a video that was recorded on June ____, 2011. I authorize New Cologne Enterprises to publish this video image of me and use it in their advertising. I hold them harmless from any damages that may be caused by the use and publication of this video."

Be sure to get contact information for these people - at least the address and date of birth.
