There is not such a form. 1244b refers to the Code of Criminal Procedure section which allows the State to prosecute what is otherwise a felony offense as a misdemeanor. The State simply moves to reduce the charge under 12.44(b) and it is usually written right onto the indictment or charging instrument and filed along with the plea papers.
