Forma Pauperis
I just received an order from the judge that stated I did not quaify to proceed in forma pauperis. And the order said I need to pay the 350.00 court cost by Nov. 17. Do I submit a new orginal petition or the petition I amended qhile my forma pauperis request was pending?

No. If you do not pay the court cost by the deadline, the court may dismiss your case, and then you would have to refile.
