What forms or paperwork is required for grandparents to file for custody of a grandchild?

You will need to file a SAPCR (Suit Affecting the Parent-Child Relationship).

If the parents are getting a divorce, you can intervene in their divorce.

If the parents agree to let you be managing conservators, it saves a lot of time and head-ache.

If the parents do not agree and at least one parent is considered to be responsible and fit, then the recent US Supreme Court case (Troxler) will probably apply. Troxler limits grandparents rights when one parent is considered fit. However, the courts are still interpreting the Supreme Court's recent decision.
