form for relinquishment of custodial rights
my sister is sole managing conservator of my granddaughter, and has recently abandoned her and left her in my care.i need a form that states she gives up her rights to the child.The father and mother have both relinquished thier parental rights.

This is not a form, but a Motion (Agreed, if possible) to Modify conservatorship. It will prompt a court order naming you as the primary conservator of the child
