My health Insurance is screwing me over

I paid about 6 months or more of premiums for health insurance(Unicare). They gaave my doctor the authorization to do surgery to confirm his finding of endometriosis. Shortly after they sent me another insurance card with another effective date. I immediately called and asked them about the new effective date. They told me it doesn't effect my original effective date. They have new computers and everyone is receiving a new effective date. Shortly after all of my primary doctor, hospital and my ob/gyn claims has all been denied for no coverage at the time of service. I want to know do I have grounds to for a lawsuit? They took money out automatically every month out of my checking. Please, email me with a response.

The short answer is yes. You are entitled to what you bought and paid for. However, where (federal or state court); when (possibly after appeal to the insurance plan); and for what (benefits owed or other damages), all depend on what kind of plan is involved (ERISA, insured or self-funded, or non-ERISA), what the policy says, and the underlying circumstances.
