Legal Question in Employment Law in Texas

I was demoted at work for not knowing a lockout area had changed .In our safety meeting we are taught that if a lockout area is changed all team members will sign off on the change.Can I be demoted because they failed to follow procedures and do as our safety meetings teach.How can it be my fault if they did not follow procedures
