Medical lien form
I need medical lien form(to be filed in the tx court house) that my patient can sign and the
third party insurance will honor it.Third party insurance
always send my payment or settle claims directly with the patient
without paying me.

It sounds like what you are wanting to do is to find a way to make the insurance company pay you directly. If that is the case, then it sounds like you want to have your patients sign an Assignment of Benefits form when you first see them. After that form is signed it would be wise to immediately send it to the insurance company in order to put them on notice of the contractual assignment.
