What type of will form should I use for domestic partner with one adult and one teenage kid

A will for a domestic partner cannot be easily answered with the limited facts here. Other factors that need to be discussed would be the size of the estate, age of the child, whether or not you would want to set up a trust and if so, the type of trust and more. I would be happy to discuss this in detail with you. Very few people set up wills but when dealing with the legal system "justice loves the prepared".

Sadly, Wills don't really come in "forms," and if you're trying to decide between a couple of forms that you've found for yourself, you should be very very careful. This is not the sort of thing that you want to leave to chance or your ability to determine why one "form" of a Will seems more appropriate for your situation than another. Contact an estate planning attorney to discuss your identified situation (and several other items) with them in order to get the ball rolling.
