Legal Question in Personal Injury in Utah

Swimming pool

I am wanting to teach swim lesson, water aerobics, and let people rent out lanes in my residential pool. I am wondering what legal forms I should have. I have check with the city and I don't have to have a business license. But wondering if I should set up a company as a LLC, Or if there is just forms that I can have partents sign.
