I was foreclosed on in utah 4 years ago. The 2nd mortgage was purchased by a collection agency who sends me a letter every 4 months. How long will this 2nd follow me? It is not on my credit report. Do I need to file bankrupcy?

The 2nd can follow you for a long time. It can be collected for at least 8 years from the last payment. If the 2nd becomes a judgment it can be collected indefinitely because the judgment can be renewed. Bankruptcy will get rid of it.
