This is really two questions: 1) who is responsible for the debt owed to the rehabilitation clinic, and 2) who is responsible for the act of writing a bad check.

The niece, if she is over 18, is responsible for her own health care expenses, unless the aunt signed a contract with the clinic where she agreed to be financially liable.

As far as possible criminal liability for writing a bad check, that belongs to the person who actually wrote the check.
