Father unfit?

What rights do i have to request that my children's interaction with their father be limited? Currently our custody agreement gives me full physical custody and he has reasonable rights of visitation. he takes them after school and random weekend DAYS. he has not kept them overnight for almost a year because he refuses to ask his girlfriend to stay somewhere else overnight (no overnight guests in order). he also smokes around the children, berates and talks down to my oldest (8) when he has a bad day at school (which is quite often, he is diagnosed ADHD and on medication), regularly forgets my son's afternoon medication and constantly feeds both boys fast food, which is not good for my 8 year old as he has bowel problem and must eat high fiber, high protein foods. I could go on and on with the distructive hurtful things he is doing to them. Also he allows his girlfriend to control too much of my children's lives. she does homework, diciplines and trys to push her religion on them. I don't want to take the children away from him altogether, but i am concerned and wish to limit their time with him.

You could file a motion with the appropriate court

for a review of the visitation issues and at the scheduled hearing make your concerns as well as proposed changes concerning the father's visitation rights known to the court at that time.
