Unemployment benefits

After reporting back to work on medical leave of absence, I was told by my employer that i was not to return back to work without a form from my doctor releasing me to work, Which i supplied them with,but they state they never recieved this information. I then supplied them with another copy of the release form through fax, and contacted them to see if they recieved the information, I recieved no response.

I am filing for unemployment and I would like to know if this will affect my unemployment benefits?

Your question is confusing as in one sentence you indicate you returned to work after having been on medical leave and in another that you're filing for unemployment benefits.

If you mean that you no longer work for the company and as a consequence, are filing for unemployment, whether you qualify for these benefits will be determined by the circumstances under which you severed your relationship with your employer, e.g. whether you were fired unjustifiably or voluntarily quit. Whether or not you returned a doctor's release form should be strictly a secondary matter and should not be

a valid basis for the employer terminating your employment, if in fact that is what occurred.
