How can I get my claim started since he is refusing to submit it to the workers

last thursday i was injured at work. I found out today that the ligment in my back is torn, and i may have a possible slipped disk. My employer has stated he refuses to submit my claim to you all and that i will have to pay for all treatment and medication received. How can I submit this claim?

Dear Sir: If your employer refuses to submit your claim to his insurance company, then you are obligated to file a claim yourself. You can call 804-367-8600, the number for the Virginia Workers Compensation Commission. You can request a Claim for Benefits form. You then fill it out and mail it back to the Commission.
