can my work require me to tell them what medication i take
My work i requiring me to fill out a form telling them what medications im on its a transportion company in wa. thay drug test if i do not tell them and it shows up on drug test can thay fire me if i have a perscription for the med?

Drug test are only permitted for certain jobs, and presumably you are working in one of those jobs (driving a truck or operating heavy equipment, for example.) If the drug you are taking will interfere with your ability to do that job, even if you are taking the meds under doctor's orders (narcotic pain killer would be an example), they can let you go.
