Guardianship of a minor

What form is appropriate to appoint guardians for a child should something happen to us, the parents? Is there something in a Will that can cover this?

Yes, this is almost always addressed in a will (or revocable trust) when there are minor children. This would be just one part of a comprehensive estate plan so discuss this with your attorney before proceeding.
