Passing of business to common law wife

What form do I need to will my half of a corporation (I am the president) to my girlfriend? What form would be used that both my business partner and I could use that would give the surviving partner sole ownership of said corporation in the event of one of our demise?

My business partner agreed before the formation of this corp. to due this with me but now refuses.

Corporate stock is typically held in the name of the owner with a designated beneficiary in case of the owner's death. That can be anyone of legal age. 2 partners would designate each other so that the survivor would be the sole owner. Any one else should be designated as the beneficiary upon the death of the stockholder.
