quit claim deed

i live in Illinois and have property in Arizona that I need to issue a Quit Claim Deed on. What kind of form do I need and how do I go about getting this form. What would it be called?

The form is a Quit Claim Deed, that is what you need. You might find one on line. Look for an Arizona Quit Claim Deed. You will probably need a form for the purchase of or waiver of Revenue Stamps from the county where the property is located. Call the County Recorder in Arizona and ask them for information.
