Each trust should be custom tailored to the individual. While there are many similarities between people, no two trusts should be alike.

Consequently, there is no single "form" to amend a trust. You didn't provide any information that would even begin to determine how you would need to amend your trust. Much will depend on what you intend to change or amend.

Before anyone could competently suggest a "form" they would need to actually review the trust agreement and get the relevant information from you.

Many tragic mistakes are made without competent counsel or with do it yourself wills and trusts. Unfortunately, most problems with estate plans are found too late. Consequently, there are atorneys that specialize in dealing with estates that are "messed up" for one reason or another.
